First Visit
Welcome to XYZ Clinic
Your first appointment will usually consist of an examination and a discussion of your treatment options. In most cases, treatment can be completed or begun on the same day as the initial consultation.
An evaluation and a second appointment to provide treatment on a different day will be required if the patient has a complicated medical history or treatment plan. It would be helpful if you could also provide the following information at the time of your consultation:

- A list of all of the medications you are currently using.
- If you have dental insurance, make sure to bring the necessary forms with you. This will save you time and enable us to assist you in the processing of any claims.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18. If you have a medical condition that may be concerning (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc. ). Also, if you are currently on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.), please notify the office before treatment.
X-Rays
If your dentist or doctor has taken x-rays, you can ask them to send them to our office. Please pick them up and bring them to our office if there isn’t enough time. Additional x-rays can be taken at our facility if necessary.
