PAYMENT OPTIONS:
Credit Card: Visa, MasterCard, Discover, or American Express.
CareCredit: Alternatively, you can apply for CareCredit either online (www.carecredit.com) or in person at our office.
PLEASE BE ADVISED THAT PAYMENT IS REQUIRED AT THE TIME OF SERVICE.
We will submit your claim to your insurance company and keep you informed. We will need to know if your insurance plan changes. Regardless of your insurance coverage, you are financially responsible for our fees. If you submit a claim to your insurance company, you can expect to hear back from them within four to six weeks. Your responsibility includes the remaining cost.
